ABOUT US

My name is Ming, and I founded Aviva Health to bring compassionate, culturally aware care to people and families across Perth.

I’m a Registered Nurse with over 15 years of experience in both hospital and community settings. I speak English, Mandarin, Cantonese, Hokkien, and Malay, which helps me support clients from diverse cultural backgrounds with comfort and understanding.

Throughout my career, I’ve learned that great care is more than treatment — it’s trust, communication, and feeling genuinely understood.

At Aviva Health, we believe everyone deserves support that feels personal, respectful, and truly connected. Our role is to walk alongside you and make everyday life easier, safer, and more enjoyable.

OUR SERVICES

  • In‑home and community support, including high‑intensity nursing care, personal care, daily living assistance, cleaning, and social participation.
  • Support for NDIS participants (self‑managed and plan‑managed), older adults receiving in‑home care, and privately funded clients.
  • We speak English, Mandarin, and Cantonese. Our team works closely with families, honouring cultural values, routines, and preferences — with a strong focus on supporting Asian communities. Families receive a weekly summary of the participant’s activities and progress.

  • Teenager to adulthood transition (ages 17–25).
    We support young adults to build independence, confidence, and real‑world skills as they move from school and family‑led care into adulthood.
    Examples: daily living and independent life skills, social participation and community access, study support, training and work‑readiness, and structured routines for neurodiverse young adults.
  • Dementia & Ageing.
    We support older adults and their families to remain safe, engaged, and supported at home.
    Examples: personal care and daily living assistance, routine‑based cognitive engagement, behaviour‑aware dementia support, meal support, medication prompts, and carer respite.
